NOTICE OF 1095-C AVAILABILITY 

In compliance with the Affordable Care Act (ACA), the Village of Carol Stream is required to provide Form 1095-C, which reflects the status of your eligibility, offer, and enrollment or waiver of Village provided health insurance coverage for the previous year. Due to the recently enacted Paperwork Burden Reduction Act, employers are no longer required to automatically distribute Form 1095-C to all employees.  Instead, these forms will be provided upon your request. 

If you would like to receive a copy of your Form 1095-C, you may make your request at any time by contacting the Human Resources Department using one of the following methods:
